Registration Requirements for Certified RN, LPN, CNA, and HHA:
- Proof of Florida State License
- CNA License in Florida or HHA certificate 40-hour minimum
- Level II Background Screening on file with AHCA
- VCPR Card through the American Heart Association or American Red Cross
- Free of Communicable Diseases dated within the last 6 months.
- HIV/AIDS Certificate – 1 Hour
- Self-Administration of Medication Certificate – 2 Hours
- Alzheimer’s Training Certificate – 1 Hour
- Social Security Card
- Proof of Residency or Citizenship
- Employer Identification Number (EIN)
- Florida Driver’s License
- Car Insurance & Registration
- Professional Liability Insurance
- II work references at least two years of experience
